The Inner North-West London HIV Service User Group

The HIV Service User Group in NHS Hammersmith & Fulham, Kensington & Chelsea and Westminster is collecting information to help inform their contribution to the continued development of services in the locality. The purpose of the HIV service user group is to ensure that HIV services in the area are influenced by the needs and views of both people living with HIV and carers of such individuals.

This survey should be completed by any person with HIV who lives or accesses services in Hammersmith & Fulham, Kensington & Chelsea or Westminster.

The survey seeks to understand how HIV Service Users in the afore-mentioned Boroughs would like to contribute to the continued development of services and in what manner they would like to achieve this. Such involvement has the potential to provide commissioners with a clearer understanding of the needs of people living with HIV.

The survey contains between thirty-five and forty short questions should take no longer than twenty minutes to complete. It is completely confidential and the information from this questionnaire will be stored on password protected computers.
